Top 10 Medical Billing Companies in California (2026), Scored and Verified

Top 10 medical billing companies in California for 2026 with a doctor and California map

The top medical billing companies in California for 2026, in order of our 100-point score, are One O Seven RCM, AllStars Medical Billing, Cosentus, MedCare MSO, Docs Medical Billing, RCM Matter, I-Med Claims, DoctorMGT, P3 Healthcare Solutions, and My RCM Group. Prices and terms come from each firm’s own website.

On October 9, 2026, we checked 33 companies besides our own and read the pricing, contract, service, and location pages on 32 of their websites; one site wouldn’t open. We scored the medical billing companies in California on one 100-point rubric and show every point in the scoring section.

Disclosure:One O Seven RCM, based in Fulshear, Texas, wrote this ranking and placed itself first. With no California address, headquarters, or California payer page, our firm scored 0 of 10 on California presence. We held every firm, ours included, to the same rubric.

California practice owners, physicians, administrators, and practice managers can use this ranking to compare outsourced billing, as can out-of-state groups billing Medi-Cal or California plans. You’ll decide who to shortlist, what to pay, what to ask, and whether to hire a local California medical billing company or a remote one.

Key Takeaways

Before you shortlist a California billing company, check five facts on price, contracts, Medi-Cal, payment timing, and filing deadlines:

  • Published starting rates among our top 10 run from 2.49% to 3% (company sites, October 9, 2026); bases vary.
  • Two of 10 ranked medical billing companies in California publish a term, notice period, and setup fee (October 9, 2026).
  • Medi-Cal covered 13.68 million people in June 2026; 94.8% were in managed care (Department of Health Care Services, preliminary).
  • State-regulated plans and insurers must pay complete claims within 30 calendar days of receipt, starting January 1, 2026 (AB 3275).
  • Medi-Cal pays 100% on fee-for-service claims it gets within six months after the month of service, then 75%, then 50% (Provider Manual, 2025).

Top 10 Medical Billing Companies in California at a Glance

Published starting rates in our top 10 run from 2.49% of collections at DoctorMGT to 3% at One O Seven RCM, and MedCare MSO lists 3 to 7% of net collections (company sites, October 9, 2026). Cosentus and P3 Healthcare Solutions don’t publish a rate.

One O Seven RCM and Cosentus are the only two firms in this list of medical billing companies in California that publish a contract term, a cancellation notice, and a setup fee policy.

Use the table as your shortlist, and confirm each price on the firm’s own site before you compare California medical billing services on cost. In our cost section, we turn these rates into monthly dollars.

Check our math yourself: the full 100-point rubric behind every score sits in How We Scored, with the points for each factor.

How We Scored California Medical Billing Companies

We scored each firm out of 100 on seven factors a buyer can check: pricing, contract terms, service scope, client reviews, accuracy, specialties, and California presence. Pricing carries 20 points because 13 of 26 billing firms we checked October 9, 2026, publish no rate; 17 publish no contract terms.

Who Made the List

A firm made the list if it runs full-service billing, shows a verified US street address on its own website, and says it serves California practices, either by naming California, Medi-Cal, or California payers or by claiming all 50 states.

On October 9, 2026, we checked One O Seven RCM and 33 other companies: 26 billing firms, six software vendors, and one hospital revenue cycle firm. Firms that failed the rule appear in the other-companies section with the reason, and our national billing company comparison covers firms in other states.

Medical billing services in California can run from any state, so remote and out-of-state firms count. A California street address still earns five points, and I-Med Claims, based in Illinois, ranks seventh among the medical billing companies in California we scored.

Medical coding companies in California that don’t submit or follow up on claims didn’t qualify. Neither did single-service specialists, such as workers’ comp lien billers, or software vendors that sell tools to your own staff.

The 100-Point Rubric

In our 100-point rubric, pricing transparency earns 20 points; contract terms, service scope, client reviews, and accuracy earn 15 each; specialty documentation and California presence earn 10 each.

The 100-point rubric we applied to every firm, One O Seven RCM included.

FactorPointsHow a firm earns them
Pricing transparency2020 for a published rate or rate range on its own site; 10 if its own pages give conflicting rates; 5 for a fee model with no rate; 0 for nothing
Contract terms1515 for term length (or month-to-month status), cancellation notice, and setup fee all published; 7.5 for one or two; 0 for none
Service scope152.5 each for eligibility and prior authorization, coding, claim submission, payment posting, denials and AR follow-up, and credentialing, when its own site names the service
Client reviews15Largest review count on Google, Trustpilot, GoodFirms, or Clutch at 4.0 or higher: 50+ = 15, 20 to 49 = 12, 10 to 19 = 9, 5 to 9 = 6, 1 to 4 = 3, none = 0; half points under 4.0
Accuracy and disclosure15Start at 15; minus 3 for each conflict between a firm’s own pages, each claim a primary source contradicts, and any undisclosed offshore work; floor of 3
Specialty documentation1010 for six or more named specialties with a consistent count; 5 for fewer than six or a conflicting count; 0 for none
California presence105 for a California street address on its own site; 3 for Medi-Cal, California payer, or California rule content on a service, location, or FAQ page (comparison articles and payer logos don’t count, ours included); 2 for a California headquarters (labeled as such, or the first or only address its site lists)

California presence tops out at 10 points because an office address matters less to medical billing in California than payer knowledge does. Your biller has to know Medi-Cal billing, Department of Managed Health Care (DMHC) plan rules, and California medical billing rules, wherever the team sits.

We counted medical billing reviews only on Google, Trustpilot, GoodFirms, and Clutch, using the US Google profile that matches the firm’s own site by name or address. Reviews a firm posts on its own website earned nothing, since no one outside the firm can check them.

Where One O Seven RCM Lost Points

One O Seven RCM scored 87 of 100 and lost 13 points: 10 for having no California address, headquarters, or California payer page, and three because its 21 Google reviews (October 9, 2026) fall in the 20 to 49 band.

Of the medical billing companies in California we ranked, One O Seven RCM is the only top-five firm without a California address. A California payer page would add three points, a California office up to seven more, and 50 or more Google reviews another three.

What We Didn’t Score

We didn’t score performance claims you can’t verify: clean claim rates, days in AR, and revenue lift. A medical billing firm can publish any clean claim rate, and we can’t check it, so we give the figure no points. We label these numbers company-reported in each profile.

Badges that a primary source contradicts cost accuracy points. We took three points off each “HIPAA certified” claim because the U.S. Department of Health and Human Services (HHS) says it doesn’t endorse or recognize private Security Rule “certifications” (see HHS guidance on certifications).

Employee reviews and Better Business Bureau (BBB) grades with no client reviews earned nothing. We used the same method for our Texas rankings, and we’ll re-check prices and scores every quarter, logging each change in the Sources, Methodology, and Updates section.

The Best Medical Billing Companies in California, Ranked 1 to 5

The five highest-scoring medical billing companies in California earned 76 to 87 points on the same 100-point rubric. Every profile uses the same 11 labels, so you can compare firms line by line, and every price comes from the firm’s own website.

1. One O Seven RCM: Best for Flat-Rate Month-to-Month Billing (87/100)

Best for: One O Seven RCM (Fulshear, TX): best for practices that want one published rate, month-to-month terms, and no setup fee.

One O Seven RCM provides full-service medical billing at 3% of monthly collections “with no add-on fees,” and that rate covers eligibility verification, claim scrubbing, denial management, payer follow-up, and monthly reporting. Credentialing runs from $107 per payer, and the team works inside your existing electronic health record (EHR).

The firm has no California office and says it serves all 50 states, so California practices get a remote medical billing partner. Its terms allow staff inside or outside the US under HIPAA agreements. The site mentions Medi-Cal only in a comparison article, so ask whether the team bills Medi-Cal plans.

  • Headquarters: 27120 Fulshear Bend Dr, Ste 900-11, Fulshear, TX 77441

  • Watch-out: no California office, and 21 Google reviews trail the counts at several California-based firms.

2. AllStars Medical Billing: Best for Tiered Published Pricing (80.5/100)

Best for: AllStars Medical Billing (Walnut Creek, CA): best for small practices that want tiered, published prices.

AllStars Medical Billing publishes three billing tiers starting from 2.99%, 3%, and 4.99% of monthly collections. Coding costs $2 per visit and credentialing $100 per payer, and setup for electronic data interchange (EDI), ERA, and electronic funds transfer (EFT) is free. Eligibility checks come with the top tier.

AllStars works from Walnut Creek and says it serves all 50 states; we found no Medi-Cal content on its site. Its open agreement says you’re “free to leave” if you’re unsatisfied, and the site lists no term or notice period.

  • Headquarters: 2121 N California Blvd, Suite 243, Walnut Creek, CA 94596
  • California presence: Walnut Creek address and headquarters; no Medi-Cal content found (7 of 10)
  • Founded: not stated on its site
  • Pricing: from 2.99% of monthly collections (three tiers)
  • Contract: open agreement; no term or notice on its site
  • Services: all six, with eligibility checks in the top tier

  • EHR systems: none named; the site cites “3rd party EHR/billing platforms”
  • Reviews: Google 5.0 from 16 (checked October 9, 2026)
  • Team location: not stated
  • Watch-out: its homepage gives two monthly growth figures (10% to 15% and 15% to 20%, company-reported); ask for a written notice period.

3. Cosentus: Best for Anesthesia and Behavioral Health Groups (79/100)

Best for: Cosentus (Irvine, CA): best for anesthesia, surgery center, and behavioral health groups comfortable with a 12-month term.

Cosentus prices its service as a percentage of net collections and keeps the rate off its site. Its FAQ spells out the terms: 12-month contracts with a 60-day exit clause, no setup fees, and no per-claim charges. Billing runs on its Medcloud platform with help from its Zeus AI agents.

Cosentus delivers RCM services in California from an Irvine headquarters and a Napa regional office. On its behavioral health page, Cosentus lists CalAIM, Medi-Cal, and county behavioral health billing, and its security program covers offshore operations centers on the same standard as its US sites.

  • Headquarters: 300 Spectrum Center Dr, Suite 1450, Irvine, CA 92618
  • California presence: Irvine headquarters, Napa office, and Medi-Cal content (10 of 10)
  • Founded: no year on its site (pages say “25+ years” and 23 years)
  • Pricing: percentage of net collections; rate: none on its site
  • Contract: 12 months, 60-day exit clause, no setup fee
  • Services: all six, including credentialing and contracting

  • Reviews: Google 4.4 from 25 (checked October 9, 2026)
  • Team location: US sites plus offshore operations centers (company-disclosed)
  • Watch-out: no published rate and a 12-month commitment; get the percentage in writing before you sign.

4. MedCare MSO: Best for Billing With Its Own EHR (78.5/100)

Best for: MedCare MSO (Irvine, CA): best for billing plus the firm’s own EHR and practice management software.

MedCare MSO lists billing at 3 to 7% of net collections on its medical billing services page, with month-to-month contracts available and no setup costs. The firm markets multi-specialty billing services and sells its own software: the HealUS EHR and the Maximus practice management platform.

On its California page, MedCare MSO says it bills fee-for-service Medi-Cal and the managed care plans that cover most Medi-Cal patients under CalAIM. The same page names Kaiser, Health Net, Anthem Blue Cross, and Blue Shield of California, and the firm also lists an office in Santa Fe, New Mexico.

  • Headquarters: 18001 Sky Park Circle, Irvine, CA 92614; also Santa Fe, NM
  • California presence: Irvine address and a California page naming Medi-Cal, CalAIM, and California plans (10 of 10)
  • Founded: “Since 2012”
  • Pricing: 3 to 7% of net collections
  • Contract: month-to-month available, no setup costs; notice period: none on its site

  • Reviews: Trustpilot 4.6 from 21; Google 4.8 from nine (checked October 9, 2026)
  • Team location: “HIPAA-Trained US Team,” per its California page
  • Watch-out: its pages give physician counts that range from 10,000 to “nearly one million,” and you’ll need to ask for the notice period.

5. Docs Medical Billing: Best for Central Valley Practices (76/100)

Best for: Docs Medical Billing (Madera, CA): best for Central Valley practices wanting a published 2.99% starting rate.

Docs Medical Billing advertises billing and coding “Starting @ Just 2.99%” without naming the base, and we found no contract term, notice, or setup fee on its site. Its services cover all six rubric areas, plus a separate dental billing division.

As a medical billing company in California’s Central Valley, Docs Medical Billing works from Madera and says it serves all 50 states. We found no Medi-Cal content on its site. Its Google profile shows 33 reviews at 5.0, the second-largest Google count among the California-based firms we scored.

  • Headquarters: 501 N E St, Suite B, Madera, CA 93638
  • California presence: Madera headquarters; no Medi-Cal or California payer content found (7 of 10)
  • Founded: not stated (BBB lists a May 2024 business start)
  • Pricing: “Starting @ Just 2.99%” (no base on its site)
  • Contract: none on its site
  • Services: all six, plus dental billing
  • Specialties: “50+ medical specialties,” with 55 listed
  • EHR systems: Epic, athenahealth, eClinicalWorks, Tebra/Kareo, and others
  • Reviews: Google 5.0 from 33 (checked October 9, 2026)
  • Team location: not stated
  • Watch-out: a company PDF calls its staff “HIPAA-certified professionals,” a credential HHS doesn’t recognize; get contract terms in writing.

Medical Billing Companies in California Ranked 6 to 10

Firms 6 to 10 scored between 57 and 75.5 points; four publish a starting rate, and none publishes all three contract terms. Before you sign with any medical billing company in California, ask for the term, the notice period, and any setup fee in writing.

6. RCM Matter: Best for a Published Rate Plus AR Software (75.5/100)

Best for: RCM Matter (Glendale, CA): best for practices wanting a published 2.99% start and an in-house AR software tool.

RCM Matter prices billing “starting at as low as 2.99% of your monthly revenue.” The firm promises “No Long-Term Contracts” on its About page but publishes no notice period or setup fee. The service list covers eligibility, coding, credentialing, denials, AR follow-up, payment posting, patient statements, and CureAR software.

RCM Matter lists offices in Glendale and Pittsburgh, and neither carries a headquarters label. PureMD Group lists the same Glendale suite, 100 N Brand Blvd, Suite 522, so ask where its billing team works. We found no Medi-Cal content on the site, and no Google profile.

  • Headquarters: 100 N Brand Blvd, Suite 522, Glendale, CA 91203; also 433 Walnut Ct, Pittsburgh, PA 15237
  • California presence: Glendale address; no Medi-Cal content found (5 of 10)
  • Founded: not stated
  • Pricing: “as low as 2.99% of your monthly revenue”

  • Services: all six areas; it names eligibility but not prior authorization
  • Specialties: 35 listed, including anesthesia, cardiology, family medicine, and wound care

  • Reviews: GoodFirms 5.0 from six (checked October 9, 2026)
  • Team location: not stated
  • Watch-out: its own pages disagree on how many EHRs it supports, and its homepage counters show no numbers.

7. I-Med Claims: Best for a Large Google Review Base (67/100)

Best for: I-Med Claims (Oakbrook Terrace, IL): best for practices that weigh client review volume when choosing a remote biller.

I-Med Claims advertises a fee “starting as low as 2.95% of the monthly collections,” and we found no contract term, notice, or setup fee on its site. The firm covers eligibility, coding, claim submission, payment posting, denial management, AR follow-up, and credentialing, and it says it has billed “since 1997.”

I-Med Claims works from Oakbrook Terrace, Illinois, and runs a California service page that names no Medi-Cal plans or California payers. Its Google profile carries 162 reviews at 4.9, the largest count among the 17 firms we scored, which earned full review points.

  • Headquarters: 1 South 450 Summit Ave, Suite 110, Oakbrook Terrace, IL 60181
  • California presence: a California service page with no Medi-Cal or payer content (0 of 10)

  • Specialties: 44 listed against a “50+” claim
  • EHR systems: none named
  • Reviews: Google 4.9 from 162 (checked October 9, 2026)
  • Team location: not stated
  • Watch-out: its FAQ says its billers are “HIPAA compliant certified,” a credential HHS doesn’t recognize, and its specialty list doesn’t match its “50+” count.

8. DoctorMGT: Best for Workers’ Comp and Personal Injury (65/100)

Best for: DoctorMGT (Anaheim, CA): best for workers’ comp and personal injury practices that need lien and med-legal billing.

DoctorMGT, the Doctor Management Services brand, quotes billing “as low as 2.49% of the collections,” and its FAQ says the final rate depends on specialty, claim complexity, and volume. We found no contract term, notice, or setup fee on its site.

DoctorMGT’s FAQ calls its “deepest expertise” California workers’ comp, and its pages cite Department of Industrial Relations (DIR) and Workers’ Compensation Appeals Board (WCAB) rules. Its services add lien negotiation, med-legal billing, personal injury billing, and credentialing to core billing. We found no Medi-Cal content.

  • Headquarters: 201 E Center St, Suite 112, Anaheim, CA 92805
  • California presence: Anaheim headquarters and California workers’ comp content (10 of 10)
  • Founded: not stated; its pages say both 15 years and 20+ years
  • Pricing: “as low as 2.49% of the collections,” varying by specialty
  • Contract: none on its site
  • Services: all six, plus workers’ comp, personal injury, liens, and med-legal billing

  • EHR systems: none named
  • Reviews: Google 5.0 from 10, under Doctor Management Services (checked October 9, 2026)
  • Team location: not stated
  • Watch-out: its own pages give different years in business, provider counts (1,000+ and 300+), and claim rates (98%, 99%, and 97%, company-reported).

9. P3 Healthcare Solutions: Best for Billing Plus MIPS Reporting (59/100)

Best for: P3 Healthcare Solutions (Ontario, CA): best for practices that want billing alongside MIPS and ACO reporting help.

P3 Healthcare Solutions publishes no price and no contract terms; its homepage promises “no hidden clauses” without a rate. Its services page lists eligibility verification, prior authorization, coding, claim submission, payment posting, denial management, AR management, credentialing, and Merit-based Incentive Payment System (MIPS) and accountable care organization (ACO) reporting.

P3 works from a single office in Ontario, California, and its California page says its billers know California rules but names no Medi-Cal plans or California payers. Its Google profile shows 52 reviews at 4.0, which earned full review points, while the missing price and terms cost it 35 points.

  • Headquarters: 3200 E Guasti Rd, Suite 100, Ontario, CA 91761
  • California presence: Ontario address and headquarters; no Medi-Cal or payer content on its California page (7 of 10)
  • Founded: not stated; its site says “7+ Years of Experience”
  • Pricing: none on its site
  • Contract: none on its site
  • Services: all six, plus MIPS and ACO reporting
  • Specialties: “40+ Specialties,” with about 44 listed, such as cardiology, nephrology, and mental health
  • EHR systems: TherapyNotes, on its mental health page
  • Reviews: Google 4.0 from 52 (checked October 9, 2026)
  • Team location: not stated; its California page mentions “California-based” specialists
  • Watch-out: its pages give two provider counts (790+ and more than 1,500), and you’ll need a quote to learn the price.

10. My RCM Group: Best for Inland Empire Practices (57/100)

Best for: My RCM Group (Chino, CA): best for Inland Empire practices that want a local office and a 2.99% start.

My RCM Group advertises “Rates Starting from 2.99%” without naming the base, and its rate page bundles daily claim submission, payment follow-up, denial management, posting, and reporting into that offer. We found no contract term, notice, or setup fee on its site.

One of several small medical billing companies in the Inland Empire, My RCM Group works from Chino and calls itself “California-based.” Its rate page lists credit bureau reporting as a patient collections question, so confirm its process follows SB 1061, which bars furnishing medical debt to credit reporting agencies.

  • Headquarters: 14125 Telephone Ave, Suite 11, Chino, CA 91710
  • California presence: Chino address and headquarters; payer logos only (7 of 10)
  • Founded: not stated
  • Pricing: “Rates Starting from 2.99%” (no base on its site)
  • Contract: none on its site
  • Services: all six, plus virtual staffing and small claims filing

  • Team location: not stated
  • Watch-out: its site calls its staff “HIPAA-Certified,” a credential HHS doesn’t recognize, and it names no specialties.

If you’re comparing these firms with your current biller, One O Seven RCM offers a free scope consultation: a 30-minute call that scopes a paid 90-day claims review, followed by a written proposal with timeline and pricing.

Other Medical Billing Companies in California We Checked

We checked 24 more firms that didn’t make the top 10: seven scored lower, five sit outside full-service billing, five didn’t meet the inclusion rule or we couldn’t confirm them, six sell software, and one serves hospitals.

Other firms we checked on October 9, 2026, with each firm’s type and the reason it sits outside the top 10.

Some medical billing and coding companies in California show up on other published lists but sit lower here. BillingParadise, Transcure, and California Medical Billing & Revenue Management lost points for conflicting prices, a missing rate, or missing contract terms, and our rubric doesn’t score service quality.

Among California medical billing companies with a local office, Human Medical Billing works from 2674 E Main St in Ventura, describes its pricing as performance-based, and holds a 5.0 Google rating from 20 reviews (October 9, 2026). It scored 53 here; we found no published rate or named specialties.

California Medical Billing Rules Your Billing Company Must Follow

Medical billing companies in California work under four rule sets: Medi-Cal (DHCS), state-regulated plans and insurers (DMHC and the California Department of Insurance, or CDI), Medicare (Noridian), and workers’ comp. Each sets its own filing deadline, and Medi-Cal cuts payment on late claims.

Medi-Cal Fee-for-Service vs Managed Care

Medi-Cal enrolled 94.8% of its 13,675,721 certified eligibles in managed care plans in June 2026, according to preliminary DHCS Fast Facts, so most patients’ claims go to a plan. Claims for the other 5.2% go to Medi-Cal fee-for-service.

Five managed care models cover all 58 counties. County Organized Health Systems run in 34 counties, Two-Plan models in 14, Regional plans in five, Single-Plan models in three, and Geographic Managed Care in two, Sacramento and San Diego, per DHCS’s 2026 rate certification report.

Medi-Cal billing through an outside company carries its own paperwork. The Medi-Cal provider manual says Section 14040.5 of the Welfare and Institutions Code requires DHCS to enroll billing intermediaries, and electronic submitters work under a submitter ID.

Providers enroll and revalidate through the DHCS Provider Application and Validation for Enrollment (PAVE) portal. The Medi-Cal Provider Manual says “providers are responsible for all claims submitted with their provider number regardless of who completed the claim,” so you want claim-level reports from any biller you hire.

Medi-Cal Timely Filing and TAR

Medi-Cal must receive a claim within six months after the month of service, per the Medi-Cal Provider Manual. For claims arriving in the seventh through ninth month, Medi-Cal pays 75%; for the 10th through 12th month, it pays 50%. Medi-Cal denies later claims unless an approved delay reason applies.

Deadlines for Medi-Cal billing count from the end of the month of service, so March 3 and March 28 visits share one six-month window. Payers that use claim adjustment reason codes flag late claims as CO-29 timely filing denials, and you’ll need proof of timely filing to overturn one.

Services that need Medi-Cal approval require a Treatment Authorization Request (TAR) before Medi-Cal pays. We cover authorization checks before claim submission in our prior authorization guide.

DHCS All Plan Letter (APL) 26-008 (May 12, 2026) requires Medi-Cal managed care plans to meet the prior authorization timeframes in 42 CFR 438.210(d), which set seven calendar days for standard requests and 72 hours for expedited ones.

Commercial Plans and the 30-Day Payment Rule

State-regulated health plans and insurers, including Medi-Cal managed care plans, must pay a complete claim within 30 calendar days of receipt for claims they receive on or after January 1, 2026, under AB 3275. Medical billing services in California need to track that clock on every commercial claim.

Late payments carry interest at 15% a year, and a plan that skips the interest owes an extra $15 or 10% of the accrued interest, whichever is greater, per DMHC guidance on AB 3275. Your biller should check every late payment for that interest.

DMHC regulates all HMOs, some PPO and EPO products, and most Medi-Cal managed care plans, covering 97% of state-regulated commercial and public enrollment (DMHC 2024 Annual Report). CDI regulates health insurance policies. Self-insured employer plans fall outside both state agencies.

AB 72 Surprise Billing

Since July 1, 2017, AB 72 has limited what a patient pays when a non-contracted professional treats them at an in-network facility for non-emergency care. The patient pays in-network cost sharing, and the plan pays the professional the greater of its average contracted rate or 125% of Medicare.

Either side can take a payment dispute to binding independent dispute resolution (IDR), which the DMHC dispute resolution process handles for plans and the CDI dispute resolution program handles for insurers. AB 72 doesn’t apply to emergency services, dental providers, or Medi-Cal managed care plans, and Medicare and self-insured plans fall outside state law.

Medicare in California

Noridian Healthcare Solutions processes Medicare Part A and Part B claims for California as the Jurisdiction E Medicare Administrative Contractor (MAC), per the Centers for Medicare & Medicaid Services (CMS) CMS Jurisdiction E page.

Medicare wants each claim no later than one calendar year after the date of service under 42 CFR 424.44, so Medicare billing services track that limit claim by claim.

Noridian posts its own coverage and billing updates, and we explain how to keep up in our post on MAC billing guidelines.

Workers’ Comp Billing

California workers’ comp bills are due within 12 months of the date of service for services on or after January 1, 2017, under Labor Code 4603.2. Claims administrators must pay uncontested amounts on electronic bills within 15 working days, under Labor Code 4603.4(d).

A provider who disputes a payment asks for second bill review within 90 days of the explanation of review (8 CCR 9792.5.5). The Division of Workers’ Compensation (DWC) explains the 12-month and 15-working-day rules in its DWC e-billing FAQ. DoctorMGT, Medrina, and Medical Lien Management focus on this work.

HIPAA and CMIA

A billing company that handles your claims is a HIPAA business associate, so you need a signed business associate agreement (BAA) before you share patient data. HHS lists billing as a business associate activity in its HHS business associate guidance.

California adds the Confidentiality of Medical Information Act (CMIA): Civil Code 56.10(c)(3) lets you share records with a billing or claims management service, which can’t pass them on in a way that breaks the CMIA. Negligent release can cost up to $2,500 per violation (Civil Code 56.36).

SB 1061 took effect January 1, 2025, and Civil Code 1785.27 now bars anyone from furnishing medical debt information to a consumer credit reporting agency. Ask any biller that runs your patient collections how its process complies.

What Doesn’t Apply in California

California sits outside CMS’s Wasteful and Inappropriate Service Reduction (WISeR) model, which adds prior authorization to some Original Medicare services from January 1, 2026, per the CMS WISeR model page. Three limits to keep straight:

  • WISeR covers New Jersey, Ohio, Oklahoma, Texas, Arizona, and Washington.
  • AB 72 doesn’t cover Medi-Cal, Medicare, self-insured plans, or emergency services.
  • Self-insured employer plans fall outside AB 3275, which binds DMHC plans and CDI insurers.

If Medi-Cal or plan denials are stacking up near these deadlines, One O Seven RCM offers denial management help with denial review and appeals.

How Much Do Medical Billing Companies in California Charge?

Of the 10 medical billing companies in California we ranked, nine charge a percentage; P3 Healthcare Solutions publishes no fee model. Published starting rates run from DoctorMGT’s 2.49% to One O Seven RCM’s 3% (company sites, October 9, 2026), and MedCare MSO’s net collections range reaches 7%.

Fee models the firms we ranked publish on their own websites, checked October 9, 2026.

Setup and exit termsFees or notice to start and stopNo setup fee: One O Seven RCM, Cosentus, MedCare MSO; Cosentus has a 12-month term with a 60-day exitNotice period, data return, and any early exit fee

Add-ons change the math. AllStars charges $2 per coded visit, and credentialing runs $100 to $107 per payer at the two top-10 firms that publish it, AllStars and One O Seven RCM. If you’d rather hire one of the credentialing companies in California, compare its per-payer price with these.

Net collections equal gross collections minus refunds and adjustments, so 3% of net costs less than 3% of gross in the same month; we explain the measure in our net collection rate post. A biller can bury medical billing and collection fees in that definition, so get it in writing.

What a Practice Pays: A Worked Example

At 3% of collections, a practice collecting $80,000 a month pays $2,400 in billing fees. We ran three published rates against three practice sizes.

Monthly billing fee at DoctorMGT’s 2.49% starting rate, One O Seven RCM’s 3%, and AllStars’ 4.99% All-Inclusive tier (company sites, October 9, 2026).

Monthly collectionsAt 2.49%At 3%At 4.99%
$40,000$1076$1,200$1,996
$80,000$1,992$2,400$3,992
$150,000$3,735$4,500$7,485

California medical billing services priced on a percentage move with your collections, so a slow month costs less in fees. The base still differs by firm: DoctorMGT quotes collections, MedCare MSO net collections, and RCM Matter monthly revenue. Credentialing one provider with five payers at $107 per payer adds $535.

One O Seven RCM offers billing at 3% of collections with no setup fee, month-to-month terms, and 30 days’ notice to cancel, so a practice collecting $80,000 a month pays $2,400.

In-House Billing Cost in California

California’s billing and posting clerks earned a mean $58,180 a year ($27.97 an hour) in May 2025, and medical records specialists averaged $68,230, according to Occupational Employment and Wage Statistics (OEWS) BLS wage data from the Bureau of Labor Statistics (BLS). The clerk figure covers billing clerks in every industry, medical offices included.

One clerk at that mean wage costs about $4,848 a month before payroll taxes, benefits, software, and cover for vacations and sick days. At 3%, the same $4,848 pays for outsourced billing on about $161,600 in monthly collections.

Before you hire a medical biller, price the whole role against outsourced medical billing services in California, including who works your claims when that person takes vacation or leaves the job.

Is Percentage Pricing Legal? The Federal Billing-Agent Rule

Medicare and Medicaid payment rules allow a percentage-of-collections fee when the billing company prepares and submits claims; the limit applies when the biller itself receives the payer’s money (42 CFR 424.73(b)(3); 42 CFR 447.10(f)).

  • Under 42 CFR 424.73, Medicare pays an agent in the provider’s name only if the agent’s pay “is not related in any way to the dollar amounts billed or collected”; Medicaid applies the same rule in 42 CFR 447.10(f).
  • In its Medicare Claims Processing Manual (Chapter 1, Section 30.2.4), CMS says those conditions don’t apply when an agent only prepares bills and doesn’t receive or negotiate the payments.
  • The HHS Office of Inspector General (OIG) warned in 1998 that percentage arrangements may raise upcoding risk in its OIG third-party billing guidance, so your contract should spell out coding oversight.

Medical claims billing services that collect payer money in their own accounts fall under the agent rule. Keep payer payments flowing into your practice’s account, and ask how your biller reviews coding before claims go out.

Myth: percentage billing fees are illegal. Fact: Medicare and Medicaid payment rules restrict them only when the biller receives the payer’s money.

How to Choose a Medical Billing Company in California: 10 Questions

Shortlist three medical billing companies in California from our at-a-glance table, then send each one the same 10 questions. You’ll learn more from their written answers on rates, terms, payment flow, Medi-Cal, and privacy than from any claimed clean claim rate, and the replies become your contract checklist.

We cover the basics for any state in our guide to choosing a billing company; these 10 questions add the California rules and the gaps we found on real sites.

  1. What is your rate, and what is it a percentage of? Rates in our top 10 apply to collections, net collections, or monthly revenue, and two firms state no base at all. Get the base in writing, along with how refunds and old AR count.
  2. Which fees sit outside that rate? Ask about setup, coding, credentialing, patient statements, and late fees. A California medical billing company that won’t put its fee schedule in writing leaves you nothing to hold it to later.
  3. What are the term, the notice period, and the exit terms? Of our top 10, only One O Seven RCM and Cosentus publish a term, a notice period, and a setup fee policy. Ask who keeps your reports and claim data after you leave, and how fast you’ll get them.
  4. Do payer payments land in our bank account? Medicare sends EFT payments only to an account in the provider’s name, under the provider’s control, per the CMS-588 EFT form. Keep commercial and Medi-Cal payments in your account too.
  5. Will you sign a business associate agreement, and where does your team work? HIPAA requires the BAA before you share patient data. Cosentus discloses offshore operations centers, and One O Seven RCM’s terms allow staff outside the US, so your contract should say where staff can work.
  6. Are you set up to bill Medi-Cal, and how do you track the six-month limit? DHCS enrolls billing intermediaries under Welfare and Institutions Code 14040.5, and Medi-Cal cuts payment to 75% after the six-month limit, so ask for the biller’s submitter ID. You also want a monthly report of claims near the deadline.
  7. Which Medi-Cal and commercial plans do you bill in our county today? Plans differ by county, from L.A. Care Health Plan in Los Angeles to CalOptima Health in Orange County. We list Medi-Cal plans for seven California regions in our region section.
  8. How do you handle late-payment interest and surprise-billing disputes? Late payments from state-regulated plans carry 15% a year interest, and under AB 72, out-of-network professionals at in-network facilities can take non-emergency payment disputes to binding independent dispute resolution (IDR). Ask how the biller spots and claims both.
  9. Can we start with a review of our current claims? With medical billing review services, you see where money leaks before you switch billers. One O Seven RCM’s 90-day claims review is a paid audit that starts with a free 30-minute scope consultation.
  10. Can you show client reviews and California references? Check review counts and dates on Google, Trustpilot, GoodFirms, or Clutch, then call two California practices the biller names. On October 9, 2026, review counts in our top 10 ranged from one (My RCM Group) to 162 (I-Med Claims).

Red flags we found on the sites of medical billing companies in California:

  • A “HIPAA certified” badge, a credential HHS doesn’t recognize
  • Different prices on different pages of the same site
  • No street address anywhere on the site
  • A clean claim rate with no named source behind it
  • Contract terms the firm won’t put in writing

If you’d like these 10 questions put to One O Seven RCM, talk with our billing team and book a consultation.

Best Medical Billing Company by Practice Type and Specialty

The best fit among medical billing companies in California depends on your payer mix more than on a specialty count. Behavioral health groups need Medi-Cal plan experience, workers’ comp practices need lien and bill review know-how, and small practices need published rates and short contracts.

Firms from our list that publish each fit on their own websites, checked October 9, 2026.

Solo and Small Practices

Solo and small practices need a rate they can budget and a contract they can leave. Eight of our top 10 publish a starting rate or range. Independent medical billing practices comparing small medical billing companies should still ask what each rate covers.

AllStars lists eligibility checks only in its 4.99% All-Inclusive tier, while One O Seven RCM includes eligibility verification in its 3%. Price the plan with every service you need, because a 2.99% basic tier can cost more than a 3% plan once you add eligibility checks.

Specialty Practices

Specialty practices depend on coding and payer rules for their field. Ask physician billing companies how many claims they bill in your specialty each month and which of your payers they already work with.

RCM services in California for anesthesia or surgery also need coders who know the specialty’s modifiers. Our specialty medical billing services page names the specialties One O Seven RCM bills, and each ranked profile gives the firm’s specialty claim where it publishes one.

Behavioral and Mental Health Practices

For Medi-Cal patients, mental health billing services in California bill managed care plans and county behavioral health programs, so ask about both. Cosentus lists Medi-Cal, CalAIM, and county behavioral health billing on its behavioral health page.

Behavioral health billing services in California also handle authorization rules that change by plan. Under DHCS APL 26-008, Medi-Cal plans must meet the federal prior authorization timeframes in 42 CFR 438.210(d). We published one Texas practice’s results in our psychiatric practice case study; the figures are company-reported.

Surgery Centers, Urgent Care, and Workers’ Comp

Ambulatory surgery centers (ASCs) bill facility claims, so ask for ASC experience; our ASC revenue cycle guide covers the basics. California urgent care billing services need same-day eligibility checks for walk-in patients.

Workers’ comp practices answer to a 12-month billing limit (Labor Code 4603.2), and DoctorMGT, Medrina, and Medical Lien Management list workers’ comp or lien billing. Claims administrators must pay uncontested e-bills within 15 working days (Labor Code 4603.4(d)), so ask any of these medical billing services in California for sample reports before you sign.

Medical Billing Companies by California Region

Most billing firms we checked that list a California street address sit in Southern California: 15 of 18, in Los Angeles, Orange, Riverside, San Bernardino, or Ventura County. Medical billing companies in California can bill from any state, so the plans a firm bills matter more than its office.

Each county’s Medi-Cal managed care model and plan names come from the DHCS health plan directory and the DHCS rate certification report for calendar year 2026. We mark Kaiser Permanente “limited eligibility” in counties where DHCS limits its enrollment.

Medical Billing Companies in Los Angeles and Ventura

Los Angeles County runs a Two-Plan Medi-Cal model with L.A. Care Health Plan and Health Net Community Solutions, plus Kaiser Permanente. If you want a medical billing company Los Angeles physicians can meet in person, RCM Matter (75.5 points) lists a Glendale suite; call ahead before you visit.

Medrina offers medical billing services in Los Angeles as a workers’ comp specialist. Among medical billing companies in Glendale, CA, RCM Matter is the only one in our top 10. Ventura County uses Gold Coast Health Plan, and Human Medical Billing (53 points) provides medical billing services in Ventura.

Medical Billing Companies in Orange County and Irvine

Orange County runs a County Organized Health System with CalOptima Health as its Medi-Cal plan, plus Kaiser Permanente (limited eligibility). Two of our top four, Cosentus (79) and MedCare MSO (78.5), count among the medical billing companies in Irvine.

DoctorMGT (65) works from Anaheim with a workers’ comp focus. Orange County Medical Billing in Westminster also offers medical billing services in Orange County, but it scored 39.5 with no published price, contract terms, or reviews we could find. BillingParadise (56.5) lists an Irvine address beside its Atlanta headquarters.

Inland Empire: Riverside and San Bernardino

Riverside and San Bernardino Counties both use the Two-Plan model with Inland Empire Health Plan and Molina Healthcare of California, plus Kaiser Permanente, which has limited eligibility in San Bernardino.

P3 Healthcare Solutions (59) and MedKarma (56) work from Ontario, My RCM Group (57) from Chino, and California Medical Billing & Revenue Management (49.5) from Rancho Cucamonga. Medical Lien Management handles workers’ comp and personal injury liens from Corona.

Medical Billing Companies in San Diego

None of the medical billing companies in California we checked lists a verified San Diego County street address. San Diego practices will work with a firm based in another county or state.

The county runs Geographic Managed Care with four Medi-Cal plans: Blue Shield of California Promise, Community Health Group Partnership Plan, Kaiser Permanente, and Molina Healthcare of California. Ask a remote biller which of the four it bills today.

Medical Billing Companies in Sacramento, CA

No firm on our list shows a Sacramento address. Sacramento is California’s other Geographic Managed Care county, with Anthem Blue Cross, Health Net Community Solutions, Kaiser Permanente, and Molina Healthcare of California as its Medi-Cal plans. Ask any biller for a sample remittance from each plan it bills.

Fresno and the Central Valley

Fresno County uses the Two-Plan model with CalViva Health and Anthem Blue Cross, plus Kaiser Permanente under limited eligibility. Docs Medical Billing (76) works from Madera next door, the closest firm on our list for medical billing for Fresno practices.

Bay Area

The Bay Area holds two of our top three firms: AllStars Medical Billing (80.5) in Walnut Creek and Cosentus, which runs a regional office in Napa.

Bay Area Medi-Cal plans differ by county: San Francisco Health Plan and Anthem Blue Cross in San Francisco, Alameda Alliance for Health in single-plan Alameda, and Santa Clara Family Health Plan with Anthem Blue Cross in Santa Clara. For Bay Area medical billing, ask how a firm handles each plan’s authorization rules.

Remote California medical billing companies can serve any county, and virtual medical billing services can work these plans from out of state, so confirm the plans a firm bills before you sign.

Frequently Asked Questions

What are the best medical billing companies in California?

The best medical billing companies in California, by our 100-point score, are One O Seven RCM (87, Fulshear, Texas), AllStars Medical Billing (80.5, Walnut Creek), Cosentus (79, Irvine), MedCare MSO (78.5, Irvine), and Docs Medical Billing (76, Madera). We scored price, terms, services, reviews, accuracy, specialties, and California presence. One O Seven RCM published this ranking.

What are the top 10 medical billing companies?

For California practices, our top 10 medical billing companies are One O Seven RCM, AllStars Medical Billing, Cosentus, MedCare MSO, Docs Medical Billing, RCM Matter, I-Med Claims, DoctorMGT, P3 Healthcare Solutions, and My RCM Group. National lists use different methods, so their order won’t match ours. We checked each firm’s website and Google profile on October 9, 2026.

How much does it cost to outsource medical billing in California?

Outsourcing costs 2.49% to 3% at our top 10’s published starting rates, on bases that vary by firm, and MedCare MSO lists up to 7% of net collections. At 3%, a practice collecting $80,000 a month pays $2,400. Ask what each rate covers, because coding and credentialing fees can sit outside it and won’t show up in the headline number.

Should you outsource medical billing in California?

You should outsource when your in-house billing costs, including unworked denials, run higher than a percentage fee. California billing clerks earned a mean $58,180 a year in May 2025, per the Bureau of Labor Statistics. A medical billing company in California must file Medi-Cal claims within six months and track whether plans pay within AB 3275’s 30 calendar days.

What services does California medical billing provide?

California medical billing covers six core services: eligibility and prior authorization checks, coding, claim submission, payment posting, denial and accounts receivable follow-up, and credentialing. Some firms add patient billing, workers’ comp lien work, or quality reporting. Ask which services a firm’s base rate includes, since some tiered plans leave eligibility checks or coding outside it.

What is Medi-Cal billing?

Medi-Cal billing means submitting claims for California’s Medicaid program, either to Medi-Cal fee-for-service or to the managed care plan a patient belongs to. In June 2026, the Department of Health Care Services counted 94.8% of members in managed care plans. So you’ll bill managed care plans such as L.A. Care Health Plan or CalOptima Health more often than Medi-Cal fee-for-service.

What is the timely filing limit for Medi-Cal?

Medi-Cal’s timely filing limit is six months after the month of service for full payment. For claims it receives in the seventh through ninth month, Medi-Cal pays 75%, and it pays 50% in the 10th through 12th month. After a year, it denies claims unless an approved delay reason applies, so don’t let unbilled visits sit in a queue.

What are California’s timely billing laws?

California’s main billing deadlines are six months for Medi-Cal fee-for-service, one year for Medicare, and 12 months for workers’ comp. State-regulated plans and insurers must pay complete claims within 30 calendar days of receipt under AB 3275, starting January 1, 2026. Commercial plans set their own filing limits in contracts, so check each one you bill.

Do medical billing companies in California need a license?

We found no California license specific to medical billing companies, but Medi-Cal enrolls billing intermediaries under Welfare and Institutions Code 14040.5. Your practice stays responsible for every claim under its provider number. We couldn’t confirm whether California’s Debt Collection Licensing Act covers billers that collect patient balances, so ask the firm before it touches collections.

Is AI replacing medical billers?

AI handles parts of medical billing, but billing staff still work claims at the firms we ranked. MedCare MSO advertises a “HIPAA-Trained US Team,” and Cosentus markets its Zeus AI agents. California’s SB 1120 has barred health plans’ AI tools from denying, delaying, or modifying care based on medical necessity since January 1, 2025; a licensed professional makes that call.

How much do medical billers get paid in California?

Medical billers in California earned a mean $58,180 a year, or $27.97 an hour, in May 2025, per Bureau of Labor Statistics data for billing and posting clerks. In the same survey, medical records specialists averaged $68,230 a year. Neither figure includes payroll taxes, benefits, or billing software, so an in-house biller costs you more than the wage.

Choosing Your California Billing Partner

Of the medical billing companies in California ranked here, shortlist three from the table, send each the 10 questions, and compare the written answers on rate, terms, payment flow, and Medi-Cal plans. A medical billing firm that answers all 10 in writing gives you something to hold it to.

Keep payer payments in your own account with any medical billing partner you choose, and get every term in writing, including how your data comes back when you leave. Medical billing services in California differ most in details you won’t find on a firm’s homepage, such as notice periods and setup fees.
